摘要:糖尿病慢性創面因長期炎癥反應、細菌感染以及組織再生能力受損而難以愈合,仍是亟待解決的臨床問題。本研究構建了一種活性氧響應型一氧化氮釋放水凝膠體系,以三羥甲基氨基甲烷改性的透明質酸為基體,引入具有天然抗菌活性的 ε-聚賴氨酸,并結合功能化中空介孔雜化二氧化硅納米顆粒,實現藥物負載與刺激響應釋放。該水凝膠可在溫和條件下快速凝膠化,形成穩定三維網絡結構,表現出良好的組織黏附性能、抗菌活性及促愈合潛能。通過結構與功能模塊的理性設計,材料在氧化應激條件下可實現異維A酸的可控釋放,并進一步觸發按需 NO 生成。體內外實驗表明,異維A酸與 NO 的釋放均呈 ROS 水平依賴性且具可調控特征,從而顯著促進創面閉合,增強膠原沉積與血管新生,并改善局部炎癥微環境。該水凝膠體系具有良好的生物相容性與智能響應特性,提示其作為多功能創面敷料在糖尿病慢性創面高效治療中具有潛在應用價值。
